The SCQ is a self-report which assesses the presence of medical problems, their severity, and whether or not the condition limits functioning (Sangha et al 2003). The medical conditions specified on the SCQ include: heart disease, high blood pressure, lung disease, diabetes, ulcer or stomach disease, kidney disease, liver disease, anemia or other blood disease, cancer, depression, arthritis, thyroid disease and back pain. Respondents have the option of adding three additional conditions.
